NHS Health Checks

The NHS Health Check programme aims to help prevent heart disease, stroke, diabetes and kidney disease. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions, will be invited (once every five years) to have a check to assess their risk of heart disease, stroke, kidney disease and diabetes and will be given support and advice to help them reduce or manage that risk.

An invitation will be sent to all eligible patients.
If you are aged between 40-75 and haven’t had an invitation yet, please contact the surgery to book an appointment.
Please ask specifically for this appointment as you will need a 20 minute appointment with our Health Care Assistant.
Please note that you will have a blood sample taken during this appointment to check your cholesterol, blood sugar and kidney function.

Following the appointment you will need to contact the surgery for results after one week.  
The receptionist will inform you if you need to see/speak to your doctor/nurse.
